Registration for Students

2012-2013 School Year

Welcome to the Warren County R-III School District! We are happy to have you join our Warrior Nation family. Our staff is committed to helping every child succeed and reach for the stars!

To enroll a student in the Warrenton School District, you and your child must be residing within the district boundaries. After determining which school your child will be attending, you may call the school to make an appointment to register your child. A listing of all schools with the contact information is listed below for your convenience. 

You will need to have the family forms filled out only once per household. Every student will need to fill out their individual student forms listed below.

Please bring the following information with you at the time of registration. 

1) Birth Certificate
Please bring your child’s birth certificate.

2) Proof of Residency
Please bring a copy of a current mortgage statement or lease and a current unpaid utility bill with your name and address on it to provide proof that you reside in Warrenton.

3)Immunization Record
Missouri law requires immunization of school children against polio, diphtheria, rubella, measles, mumps and Hepatitis B. All students must meet immunization requirements before they may attend school.

4) Grade Card 
Please bring a copy of the last grade card your son/daughter received from the last school he/she attended.  

5) Photo Required
Please bring a current photo ID of parent/guardian at time of registration.

Residency Requirements

Under Missouri law (RsMO167.151), a student must live within a school district in order to attend that district's public schools without charge. A student may not establish residency away from a legal guardian or parent for the sole purpose of enrolling in a district without the obligation of paying tuition. In order to establish residency in the Warrenton School District, the parent or legal guardian must provide the following documents in his/her name to the school where the child will be attending:

• A copy of a current mortgage statement or lease, or
• A current unpaid utility bill.

If the parent cannot provide these documents because of special living arrangements, the parent must call (636)456-6901 to make an appointment to register the child in the Warrenton School District.

 

 


Immunization Requirements
 

Missouri State Law Bill Number 167.181

Missouri law requires that students meet immunization requirements on the FIRST DAY of school. These would include all students unless noted otherwise:

l. Diphtheria (DTaP/DTP, DT or Td)
A minimum of four doses is required for students in grades kindergarten through 10th grade and three doses required for those in grades 11 through 12. One of the three doses must be on or after the child's fourth birthday. An additional dose of Td is required 10 years after the last dose. Td booster may be given five years after last DTaP/DTP. Repeat every 10 years throughout life.

2. Polio (TOPV)
A minimum of three doses is required of Trivalent Oral Polio vaccine. One of the three doses must be on or after the child's fourth birthday. (If a combination of IPV/OPV is received, four doses are required.)

3. Rubella (German Measles or Three-Day Measles)
Kindergarten through 6th grade requires two doses with 28 days between doses with one dose required on or after the child's first birthday. For children in grades 7 through 12, one dose is required, received on or after the child's first birthday.

4. Measles (Rubeola, Hard or Red Measles)
Two doses must be administered at least 28 days apart on or after the child's first birthday, grades K-12.

5. Mumps (MMR)
Kindergarten through 6th grade requires two doses with 28 days between doses with one dose required on or after the child's first birthday. For children in grades 7 through 12, one dose is required, received on or after the child's first birthday.

6. Hepatitis B
Three doses of Hepatitis B vaccine are required for all students in grades through 12. Students ages 11-15 may use the following as a guide for the two-dose Merck Recombivax Hepatitis B vaccine schedule: Dose two, four to six months after dose one.

7. Varicella (chickenpox)
1 dose or proof of disease K-4th grade.

Missouri School Law requires that children who are not completely immunized will not be allowed to enroll in school and will be excluded immediately. If you have any questions regarding your child’s immunization status or for more detailed information, please check with your school nurse.
 

NEW IMMUNIZATION REQUIREMENTS FOR 2012-2013 SCHOOL YEAR

The Missouri Department of Health and Senior Services recently revised the code of state regulations, immunizations requirements for school children.

Tdap (tetanus, diptheria, and pertusis) vaccine will BE REQUIRED for all incoming eighth grade students if the child has completed the recommended childhood Dtap/DTP vaccination series and has not received a
TD booster within the past two years.


The intent of this article is to inform you in advance that in order for your child to start classes on the first day of 2010-2011 school year (in August), they must have received the Tdap immunization and turned in the paperwork that provides proof that they have received the immunization. Failure to do so will result in expulsion from school until this requirement is met.

Since this is a new law, there will more than likely be a backup for appointments at either your personal physician or the local health department. By getting your child's immunization done in advance  (before August, 2010), you will avoid having problems getting an appointment and/or starting the first day of school. Please be reminded that when you decide to get this done (March-August) the required paperwork needs to either be turned in or mailed to the nurse's office at BHMS. The nurse will then cross your child's name off the list and they will be allowed to begin the first day of school.

Early Childhood Requirements

The following explains what age-appropriate immunizations are required for preschool attendance to be in compliance with the law. Required immunizations by the time a child reaches the upper-age limit are:

0-14 months- 3 DTP, 2 IPV, 1 Varicello
15-17 months - 4 DTP, 3 IPV, l MMR
18 months to school entrance - 5 DTP, 4 IPV, 2 MMR, l HB (HbCV)
0-18 months - 4 HB

One dose of Varicella (chicken pox) vaccine on or after the child’s first birthday is required.
